Checklist

Below is an itemized 'Checklist' of requirements that must be completed before any animals can be ordered from the Division of Laboratory Resources (DLAR, 412-648-8950).

To obtain more detailed information about each requirement in the quicklist, click on the item in the left panel of this web page.

Requirements:

1. Enroll in the Animal Exposure Surveillance Program (AESP)

The AESP is utilized to identify and enroll animal users in preventive medicine programs and to provide occupational health information related to the use and care of animals at the University. A description of the program (PDF) and enrollment forms can be downloaded from the Environmental Health & Safety web site.

2. Create an HS Connect Account Online
The HSConnect account allows access to Institutional Training Module web sites.

3. Complete the annual Conflict of Interest Superform Online
Use your HSConnect login to gain access.

4. Complete "Research Integrity Training" Online
(Formerly RPF Module 1). Use your HSConnect login to gain access.

5. Complete "Use of Laboratory Animals in Research & Education & Training" Online
(Formerly RPF Module 3)

This online training course also includes the mandatory 'Chemical Hygiene Training' (formerly RPF Module 10). Use your HSConnect login to gain access.

6. Complete Species-Specific Training:

For investigators using rodents in their research, complete online "Small Animal Research Training" (Formerly available by videotape only; use your HSConnect login to gain access).

For investigators employing large animals (rabbit, ferret, cat, dog, pig, sheep, goat, and calves) and nonhuman primates, videotape training is required. Contact Julie Cursi at jac73@pitt.edu or 412-383-1737.

7. Submit an "Application for Approval to Use Vertebrate Animals in Research"*
*must be accompanied by an Environmental Health & Safety Agent Registration Workbook.

First: download a "New Application" form from the IACUC web site.
Email completed form to iacuc@pitt.edu

Second: download the "Agent Registration Workbook" from the Environmental Health & Safety web site. Email the completed workbook to BioSafe@ehs.pitt.edu

8. Check For Animal Housing Availability
Contact the attending veterinarian at 412-648-8950.

9. Obtain Appropriate Security Access Cards for Animal Housing Facilities
Contact DLAR at 412-648-8950.

10. Arrange a Tour of the Appropriate Animal Housing Facility
Contact DLAR staff at 412-648-8950.

11. Request DLAR to Order Animals from Approved Vendors
Call DLAR at 412-648-8950 for a list of approved vendors.

12. Ensure that Vaporizors, Gas Anesthesia, and Monitoring Equipment are calibrated and certified
The IACUC revised gas anesthesia policy and a listing of service providers (including telephone numbers) can be found at the IACUC web site.

Additional Requirements (If Applicable):

13. Complete "Human Embryonic & Fetal Stem Cell Research Training" Online
(Formerly RPF Module 5; use your HSConnect login to gain access)

14. Complete "Bloodborne Pathogens Training" Online
(Formerly RPF Module 9; use your HSConnect login to gain access)

15. Complete "Chemical Hygiene and Formaldehyde Awareness Training Session" Online held by Environmental Health & Safety.
(Formerly RPF Module 10; use your HSConect login to gain access)

16. Attend and Complete a live "Isotope Research Safety Training Session"
held by the Radiation Safety Office.

17. Complete Form RSO-313 " Authorization for Non-Human Use of Radionuclides."
Required to obtain radioisotopes for non-human use by the Radiation Safety Office.

18. Complete Form RSO-AN1 "Use of Radioactivity in Animal Studies."
Required before any radioisotopes can be administered to animals by the Radiation Safety Office.

19. Complete Conflict of Interest Form Training Online; if applicable
(Formerly RPF Module 4; log in via HSConnect).

20. Complete an Institutional Biosafety - Recombinant DNA Application
(if applicable).

21. Obtain a DEA License for the Use of Controlled Substances in Animals
( if applicable).

22. Submit a Material Transfer Agreement Form
