Animal Exposure Surveillance Program (AESP)
Enrollment in the Animal Exposure Surveillance Program (AESP) of the University of Pittsburgh is designed to protect individuals and/or minimize the affects of known hazards associated with working with animals, including infectious diseases and animal allergies.
Enrollment in the AESP is accomplished by completing a health questionnaire and undergoing a clinical assessment.
First: Complete the "AESP Enrollment Form" (a.k.a. "pre-placement Health Questionnaire) Revised 11/2007 (pdf format file).
Second: Complete a clinical assessment (required of all University of Pittsburgh employees who work with animals) at the Employee Health Services Clinic, 3708 Fifth Avenue, Medical Arts Building, Suite 500.59, Pittsburgh, PA 15213. Hours are between 7:30 and 3:00 pm Monday through Friday.
The AESP program is updated annually. The Environmental Health & Safety Department will send an "AESP Update Form" Revised 3/2007 (pdf format file) to you each year. Call Dr. Yolanda Lang of Employee Health Services for more information (412-647-1993).
The Environmental, Health & Safety Department wants to assure all individuals enrolling in the Animal Exposure Surveillance Program that all medical information will be handled with the strictest confidence and in compliance with all applicable regulations.
